Liability Waiver
- I will receive information and instruction while participating in the class, health program or workshop offered by Perfect Posture Pilates. I recognize that this class will require physical exertion, which may be strenuous and may cause physical injury and I am fully aware of the risks and hazards involved.
- I understand that it is my responsibility to consult a physician prior to and regarding my participation in this class or any other activity associated with Perfect Posture Pilates. I represent and warrant that I am physically fit, and I have no physical conditions, which would prevent my full participation in the class, health program or workshop.
- I agree to assume all responsibility for any risks, injuries or damages, known or unknown which I may incur as a result of participating in the program.
- I knowingly and voluntarily and expressly waive any claim that I may have against Perfect Posture Pilates for injury or damages that I may sustain as a result of my participation.
- Heirs, my legal representatives or I, forever release and waive any liabilities against Perfect Posture Pilates and its instructors for any injury or death incurred by my voluntary participation in this class, workshop or activities.
I have read the above release and waiver of Liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
Studio Policies:
- All classes require a pre-registration and pre-payment.
- All Sessions start and end on time. Sessions are 55 minutes.
- Private, Duet- sessions and ( Reformer Trio, when offered ) have a 24-hour cancellation policy. Failure to cancel within the 24-hour window will result in a charge for that session. Mat classes have 12 hour cancellation window.
- All packages are non refundable and non transferable.
- We accept cash, and any major credit cards (Visa or Master card, Amex, Discover).
- All payment transactions are subject to a 4.5% city sales tax.
- All private and duet sessions are by appointment only.
- All packages have an expiration date
- We reserve the right to substitute instructors without notice.
- Classes and trainer rates are subject to change.
- In order to keep our space a place of peace, please turn off cell phones prior to entering the studio
- Please refrain from wearing heavy perfumes, colognes and scented lotions.
- No shoes are allowed in the studio. Please take them off as soon as you come in.
- No client will be admitted to a group class 10 min after the start time. The full price of the class will be charged.
- Perfect Posture Pilates uses all Balanced Body Equipment.
